HNHB List of Adult Day Programs

Eligibility Criteria

You must...

  • be 18 years or older

  • have a valid Health Card number

  • NOT live in a Long Term Health Care Setting

Suitability Criteria

  • be able to tolerate a group setting

  • be medically stable

  • be able to safely perform a one-person assisted transfer,  if assistance with transfers is required during the program.  Please note a few programs  accommodate  heavier transfer needs; consult further with programs.

  • not pose a risk to self or others

Once a vacancy arises in your selected Adult Day Program, you will be contacted and enrolled in the program.
